Semaglutide or Tirzepatide for Obesity

What is semaglutide and how can it help with obesity?

Semaglutide belongs to a class of medications known as glucagon-like peptide-1 (GLP-1) receptor agonists. It mimics the GLP-1 hormone that is released in the gastrointestinal tract in response to eating. One role of GLP-1 is to prompt the body to produce more insulin, which reduces blood glucose (sugar). GLP-1 in higher amounts also interacts with the parts of the brain that reduce appetite and signal a feeling of fullness.

There are currently three FDA-approved semaglutide products: Ozempic, Rybelsus, and Wegovy.

What is tirzepatide and how does it help with obesity?

Tirzepatide activates receptors of hormones secreted from the intestine (glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP)) to reduce appetite and food intake.

There are currently two FDA-approved tirzepatide products: Mounjaro and Zepbound.

How does semaglutide help with weight loss?

Semaglutide works by mimicking a hormone in your body called glucagon-like peptide-1 (GLP-1), which regulates appetite. By increasing the levels of this hormone, semaglutide can help reduce feelings of hunger and increase feelings of fullness, leading to a reduced calorie intake and weight loss.

How does tirzepatide work in promoting weight loss?

Tirzepatide works by imitating the actions of two different hormones in the body, GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). These hormones play a key role in managing your blood sugar levels and appetite. Tirzepatide can help reduce your calorie intake by making you feel fuller, and thus, promotes weight loss.

Semaglutide - How much weight loss?

Patients can expect to lose 15% of their body weight in the course of treatment. The amount of weight loss can vary depending on the individual, the dosage, and adherence to a healthy diet and exercise regimen.

How are these meds administered for weight loss?

Semaglutide and tirzepatide are medications that are administered as a once-weekly injection. At WLNS, we will instruct you on how to administer the medication to yourself.

Tirzepatide - How much weight loss?

"Tirzepatide is the first investigational medicine to deliver more than 20 percent weight loss on average in a phase 3 study, reinforcing our confidence in its potential to help people living with obesity," - Jeff Emmick, MD, Ph.D. A phase 3 clinical trialfound a high dose of tirzepatide helped patients lose 22.5% of their body weight on average.

How long does it take for these meds to promote weight loss?

The effect of semaglutide or tirzepatide can be seen as early as the first few weeks of treatment. The effect of varies among individuals and the amount of weight loss can also depend on adherence to a healthy diet and exercise regimen.

Are there any side effects associated with these meds?

The most common side effects of semaglutide or tirzepatide include nausea, vomiting, diarrhea, decreased appetite, and abdominal pain. Some individuals may experience more serious side effects. If you have any concerns or if your side effects persist or worsen, reach out to your healthcare provider immediately.

Can anyone use semaglutide or tirzepatide for weight loss?

Not everyone is a candidate for semaglutide or tirzepatide. It's typically recommended for adults who are struggling with obesity or overweight with at least one weight-related medical condition, like hypertension or type 2 diabetes. It's not recommended for patients with a history of pancreatitis or diabetic retinopathy. Always consult with your healthcare provider before starting a new medication.

In special circumstances, semaglutide or tirzepatide may be prescribed as an off-label use for weight loss management in patients who are overweight or obese without weight related issues such as hypertension or type 2 diabetes.

WLNS can work with you to see if you are a candidate for these medications. If either semaglutide or tirzepatide are not a good fit for you, we have other options and will work with you to help you acheive your weight loss goals.

How is compounded semaglutide or tirzepatide different from regular tirzepatide?

Compounded versions of semaglutide or tirzepatide are specifically made to fit an individual patient's needs, as prescribed by their healthcare provider. This could involve changing the form of the medication, adjusting the dosage, or combining it with other medications. Regular tirzepatide comes in a standard dosage and form.

Are there any contraindications to semaglutide or tirzepatide?

Important Safety Information Wegovy (semaglutide) injection is a prescription medicine for patients with obesity (BMI ≥30) or overweight (BMI ≥27) who also have weight-related medical problems, to help them lose weight, paired with diet and exercise. Wegovy is semaglutide and should not be used with other semaglutide-containing products or other GLP-1s. It is not known if Wegovy is safe and effective when taken with other weight loss products. It is not known if Wegovy can be used safely in people with a history of pancreatitis. Wegovy may cause serious side effects including possible thyroid tumors and thyroid cancer. Do not use Wegovy if you or any of your family have ever had a type of thyroid cancer called medullary thyroid carcinoma (MTC) or if you have an endocrine system condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Wegovy may cause other serious side effects, including pancreatitis, gallbladder problems, increased risk of low blood sugar, kidney problems, serious allergic reactions, changes in vision in patients with type 2 diabetes, increased heart rate, and depression or suicidal thoughts.

Tirzepatide is contraindicated in patients with a personal or family history of MTC or in patients with multiple endocrine neoplasia syndrome type 2 (MEN 2). Counsel patients regarding the potential risk for MTC with the use of tirzepatide and inform them of symptoms of thyroid tumors (eg, a mass in the neck, dysphagia, dyspnea, persistent hoarseness). Routine monitoring of serum calcitonin or using thyroid ultrasound is of uncertain value for early detection of MTC in patients treated with tirzepatide.

Compounding Pharmacies

What is a compounding pharmacy?

A compounding pharmacy is a specialty pharmacy that makes medications tailored to individual patient needs. This can involve altering the form of a medication, removing certain ingredients due to allergies, or creating a dosage that is not commercially available.

Why would I need a compounding pharmacy?

You might need a compounding pharmacy if you have specific medication requirements that aren't met by commercially available drugs. For example, if you're allergic to a certain ingredient in a standard medication, a compounding pharmacy can create a version of that medication without the allergen.

If there is a drug shortage of a patented medication, then the FDA allows compounding pharmacies to make compounded versions of these medications. Currently semaglutide and tirzepatide are on the FDA drug shortage list so they are allowed to be made by compounding pharmacies.
